Responsible for processing all orders for Permanent Change of Station (PCS) and Temporary Change of Station (TCS)

Levy Packets are required to process PCS orders

 

All Levy Packets must contain the following:

 

DA Form 31 

DA Form 5118

DA Form 5434

Commander Memorandum

TDY Option Statement (If TDY)

 

Additional information must be included in Levy Packets 

 

OCONUS to CONUS

 

DA Form 4787 (Soldier’s with dependents)

 

OCONUS to OCONUS

Travel of dependents to gaining OCONUS PDS is only authorized after Command Sponsorship is granted by Gaining Command.
